Objective: The purpose of this study was to determine the relationship of periapical lesions in upper molars with the thickening of the antral mucosa of maxillary sinuses evaluated by Cone Beam tomography. Materials and Methods: The sample consisted of 80 Cone Beam tomographies from the Diagnostic Imaging radiological center. A total of 320 upper molars were selected, of which the thickening of the antral mucosa was evaluated according to the presence or absence of associated periapical lesions. Taking as anatomical parameters to measure the apicosinus distance, the uppermost part of the apex of the upper molars and the lower part of the maxillary sinus. Results: The thickening of the antral mucosa of the maxillary sinuses in presence, absence, appearance and severity is associated in upper molars with periapical lesions. Being the P-Value 0.001 and at a signi-ficance level of 0.05. This allows us to reject the null hypothesis. Conclusions: Periapical lesions in upper molars evaluated by Cone Beam tomography are significantly related to the thickening of the antral mucosa of the maxillary sinuses.
